You For Life offers the following services

  • Private Consultations - fees apply 

  • Medicare Chronic Disease Management (CDM) Consultations - bulk billed (no gap fees) for eligible patients up 5 sessions, regular review session fees will apply after this*

Private Consultations

A referral from a general practitioner (GP) is not required to access this service. You may be able to claim a rebate from your Private Health Insurance depending on your policy. Please check with your provider for more details.

Chronic Disease Management (CDM)

If you have a chronic disease you may be referred by your GP under CDM plan where you may receive up to 5 bulk billed consultations per calendar year to see an allied health professional. Please discuss you eligibility with your GP. 

Examples of chronic disease include diabetes, heart disease, high cholesterol, high blood pressure and more.

Initial Consultation

During your first session we will explore the following:

  • Your health concerns

  • Lifestyle

  • Health and medical history

  • Baseline measurements

  • Diet history

 

Together we will make a plan​ and set some goals.

Small changes overtime enable sustainable lifestyle choices. 

Review Consultation

Follow-up sessions will monitor recommendations and your goals

Group Session

Small group/couple sessions on healthy eating

  • Information on what is healthy eating

  • Tips on how to apply the information

  • Group brainstorming in a non-pressured environment

Individual or small group supermarket tour to help navigate how we can healthier choices

  • Tips on label reading

  • Tips on how to navigate a supermarket 

  • Choosing healthier food products
